Talkspace and Better U both treat depression and anxiety online, but they sit at very different points on the treatment path. Talkspace offers first-line care: talk therapy and psychiatry, often covered by insurance. Better U offers at-home ketamine therapy — an off-label, self-pay option intended for people for whom standard treatment hasn’t worked.
The short answer
- Start with Talkspace (or a similar service) if you haven’t tried therapy and standard medication.
- Consider Better U only if you’re an adult whose symptoms haven’t improved after standard treatment, and after discussing it with a clinician.

A note on ketamine
Generic ketamine is FDA-approved as an anesthetic; prescribing it for depression or anxiety is off-label. The related drug esketamine (Spravato) is FDA-approved for treatment-resistant depression, but only under supervision in a certified clinic. At-home ketamine can raise blood pressure and cause dissociation, long-term evidence is still limited, and in 2023 the FDA warned about risks of compounded ketamine used at home for psychiatric conditions. Better U’s screening excludes many people for safety reasons — see our Better U review for the full list.
Using them together
Ketamine programs typically work best alongside ongoing therapy. Better U includes integration coaching, but it isn’t a substitute for regular therapy with a licensed therapist — something Talkspace can provide.
Our verdict
Talkspace is the appropriate starting point for most people. Better U is a specialized option for treatment-resistant symptoms, with real costs, eligibility limits and safety requirements to weigh with a clinician.
